***AGES 17 & UNDER IN PITS: Anyone entering the pits age 17 and under needs to have this release form filled out and notarized. DSS needs to have a copy of two forms of ID on file (Ex: State Issued ID, birth certificate, social security card, passport...). This has to be DONE before they can enter the pit area! Open each of the 2 following pages, right click the form to save or print. Page 1 - Page 2
